Take Route 495 (Long Island Expressway) to Exit 49 (East from New York, West from Eastern Long Island).  Go South on Route 110.  Make a right on Louden Avenue (2nd Traffic Light after Route 27).  Continue on Louden Avenue about 1/4 mile.  Broadlawn Manor Nursing and Rehabilitation Center is located on Right Side.

Southern State Parkway:

Take Southern State Parkway to Exit 32 (East from New York, bearing left at fork upon exiting, West from Eastern Long Island.) Go South on Route 110.  Make a Right on Louden Avenue.  Continue on Louden about 1/4 mile.  Broadlawn Manor Nursing and Rehabilitation Center is located on Right Side.
