The caring and compassionate professionals of Broadlawn Manor’s Admission team provide guidance to individuals who would like to participate in the Adult Day Health Program.

The Adult Day Health Program’s Admission office can be reached by calling (631) 608-5650, and is staffed from 8:00 am to 4:00pm to handle inquiries. We also invite you to contact us to schedule a tour of our facility or a trial day.

Eligibility for the Day Programs:

Determination of eligibility is based on physical, psychological and psychosocial impairments that require supervision or medical intervention. Eligibility is also determined by the need for rehabilitative services to restore or maintain optimal level of functioning. Admission to the program is also based on the client’s ability to benefit from personalized care, socialization and therapeutic recreational activities.
